1. Underline the name by which the student is officially known if more than one principal name (Surname) is entered.
2. Two passport size (5Omm x 5Omm) photographs are to accompany the Certificates. The reverse of each photograph is to bear the name of the student concerned. The second photograph is used for UK Identity Cards.
3. Delete those that do not apply and circle (paragraph 12) the relevant individual country security classification or protective marking that corresponds to the UK protective marking listed below:
TOP SECRET - Information and material, the unauthorised disclosure of which would cause exceptionally grave damage to the effectiveness or security of UK or allied forces.
SECRET - Information and material, the unauthorised disclosure of which would cause serious damage to the operational effectiveness or security of UK or allied forces.
CONFIDENTIAL - Information and material the unauthorised disclosure of which would be damaging to the operational effectiveness or security of UK or allied forces.
RESTRICTED - Information and material the unauthorised disclosure of which would make it more difficult to maintain the operational effectiveness of UK or allied forces.
4. The 'Authorising Officer' must be an official of the country named in paragraph 6 of the Certificate and his signature is to be authenticated by the official stamp of the Service Ministry concerned.
5. When required, the certificate is to be completed and forwarded in DUPLICATE.

INDEMNITY FORM 7
INDEMNITY TO PROTECT THE MINISTRY OF DEFENCE WHEN A NON-MOD PERSON ATTENDS A COURSE OR UNDERTAKES ELECTIVE STUDY AT AN MOD ESTABLISHMENT
The following indemnity is to be incorporated into an insurance policy taken out by the student (with a UK insurance company or through a UK insurance broker naming the Secretary of State for Defence, his servants or agents as the insured party and paid for by the student. Cover should be for a minimum of £500,000 (Five Hundred Thousand Pounds) for any one incident. Evidence must be produced before commencement of the course that the required insurance has been arranged (the policy, or a copy of it, and receipt for the premium paid):

Note: As part of certain training courses, students may be required to visit other Crown premises, firms, organisations or other non-MOD establishments. The insurance will be deemed to extend to cover the insured for legal liability to the student that may arise in connection with such visits. (Such liability to include any which arises due to requirement for MOD to indemnify any of aforesaid firms, organisations or establishments whose premises are visited.

FORM OF APPLICATION FOR A NON-MOD PERSON SPONSORED BY A COMMERCIAL ORGANISATION TO ATTEND A COURSE AT A MOD ESTABLISHMENT
To the Secretary of State for Defence
IN CONSIDERATION of your agreement to accept

being duly authorised to execute this agreement on their behalf HEREBY UNDERTAKE AND AGREE as follows:
1. To pay on demand the cost, as determined by the Secretary of State for Defence (hereinafter referred to as “the Secretary of State”), of the training course provided. (Delete if not applicable).
2. To indemnify the Secretary of State (as defined below) against all liability however arising in respect of any loss, damage or injury (whether or not resulting in death) and whether in property or person which may be suffered:
a. By Trainee resulting from attendance at the said course and against all claims in respect thereof.
b. By any other person by reason of any act or omission by the Trainee during the period of the said course, and against all claims in respect thereof.
3. To reimburse the Secretary of State in respect of any payment which after consultation with the sponsor’s insurers, may be made to the trainee or their representatives or to any other person or persons whether by way of compensation or indemnity and whether gratuitously or otherwise in respect of any such loss, damage or injury as aforesaid.
4. Not to prefer against the Secretary of State or without the consent of the Secretary of State any claim or loss of services or any other loss or damage howsoever arising which may be suffered by the Sponsor in consequence of any act or omission of or in relation to the Trainee during the said period.
5. To effect with an Insurance Company or Companies a policy or policies of insurance covering all the matters which are the subject of the indemnities and undertakings herein contained in the sum of £500,000 at least in respect of any one incident and unlimited in total, provided always that this provision shall not be construed as limiting or affecting in any way the Hirer’s liability under any of the said indemnities or undertakings:
a. To assign to the Crown all sums hereafter to become due under the said policy or policies and to ensure that every such policy is endorsed in the policy terms:
“It is hereby declared or agreed that notwithstanding anything contained in this policy or in any memorandum condition or schedule attached to or forming part of this policy, this policy covers all sums within the sum assured which the insured shall become liable to pay under an undertaking to the Crown, a copy of which undertaking is set out at the foot hereof”.
b. To duly pay the premium or premiums payable in respect of the said policy or policies and receipt or receipts
for the premiums for inspection whenever so required by or on behalf of the Secretary of State.
6. In this undertaking “Secretary of State” includes any Department of the Crown in the United Kingdom and any Minister, Officer, servant or agent of the Crown, the expression “claims” includes claims made by executors, administrators, successors and dependants.
In the event of a national emergency, as to the occurrence of which the Secretary of State may discontinue the service hereby provided for without notice.
